Eligibility
- Age 55 and older.
- Have difficulty with any ADL (activity of daily living), such as bathing, dressing, grooming, eating, walking across a small room.
- Cognitively intact.
- Willing to participate in goal setting and motivated to achieving client-driven goals.
- Monthly income of less than $2,000 (minus medical expenses).

For those who meet the eligibility requirements, but exceed the income level, Steward Center for Palliative Care has created CAPABLE +, a paid program that allows for the same benefits as the traditional program, including occupational therapy, nursing management, and customized handiwork.
Cost:
- $1000 per month for 4 months, billed monthly.
- First month payment due at the end of first visit (includes $500 credit towards purchase of DME/home modification materials).
- Total base cost for CAPABLE+ is $4000. The cost of handiwork is not included in the base cost.
- Pay by cash, check, or card.
